Immunizations
Our district abides by Arizona state laws regarding immunizations for school-age children. Children must be current on immunizations in order to register. Read more about Arizona immunization laws in the links below:
Enrollment
We are happy to welcome your child to our schools! If you are enrolling a child to Ganado schools for the first time, please complete the pre-registration form below. Pre-registering is just the first step.
Pre-Registration - This form is just basic information that will be reviewed before the actual enrollment process begins. Once completed, you will be sent an email with further instructions. (Please click on the link below.)
The documents listed below must be submitted to the school for enrollment. The forms on the right side (under State Forms menu) must also be completed and submitted as well.
- Your child's immunization records (Due annually)
- Your child's original birth certificate
- Your child’s Certificate of Indian Blood (CIB) - Parent may provide their CIB if child does not have one as of yet.
- Proof of residency (Electric bill, phone bill, lease/rental, drivers license if physical address is indicated with guardian's name)
- Withdrawal form from previous school
- Transcripts/Grades from the previous school
You may upload these documents through your parent account once it has been set up with instructions we send you, or you can email the documents to leola.thompson@hornet.land.
Returning Students (Students who have attended Ganado Schools before - *Not to be confused with students have completed the 2023–2024 school year at Ganado USD and will be continuing in SY 2024–2025.
